The claim
“Cold water treats depression and anxiety.”
Stretched. No full-size randomized trial has tested cold water as a treatment for diagnosed depression or anxiety. In the closest one, cold showers with Wim Hof breathing helped women with high depressive symptoms no more than warm showers with slow breathing.

The research found this
No completed, full-size randomized trial has tested cold-water immersion or cold showers as a treatment for diagnosed depression or anxiety. The closest randomized test enrolled 84 otherwise healthy midlife women in the US with high stress and high depressive symptoms.Source 1 · Randomized trialA randomized controlled clinical trial of a Wim Hof Method intervention in women with high depressive symptomsBlades R, Mendes WB, Don BP, et al.. Comprehensive Psychoneuroendocrinology, 2024.Read the paper ↗PubMed ↗ For three weeks, one group did a daily 15-minute audio-guided session of Wim Hof-style breathing plus cold showers, and the other did slow breathing plus warm showers.
Both groups improved about equally, with depressive symptoms down 24% and anxiety down 27%, and the gains held at three months.Source 1 · Randomized trialA randomized controlled clinical trial of a Wim Hof Method intervention in women with high depressive symptomsBlades R, Mendes WB, Don BP, et al.. Comprehensive Psychoneuroendocrinology, 2024.Read the paper ↗PubMed ↗ The researchers had registered a prediction that the Wim Hof group would do better, and the data did not support it. The Wim Hof group did ruminate less after daily stressors, and all six dropouts came from that group.

The trial was built to test whether a full study could run, and it was too small to show whether swimming works.
The rest of the evidence is thinner. A 2018 case report described a 24-year-old woman whose depression had not responded to two antidepressants. After she took up weekly open-water swimming her symptoms eased gradually and she came off medication, and a year later she was still off it.Source 3 · Case reportOpen water swimming as a treatment for major depressive disordervan Tulleken C, Tipton M, Massey H, et al.. EvidenceWhat kind of evidence exists in people?

ConsistencyHas the finding held up?
18/25The feasibility trial favored swimming with medium to large effects, and the bundled cold-shower trial found no advantage over its control.Source 2 · Randomized trialOUTSIDE: OUTdoor Swimming as a nature-based Intervention for DEpression: a feasibility randomised controlled trialMassey H, Denton H, Burlingham A, et al.. MagnitudeDoes the claim match the size of the effect?
22/25The word treats implies a tested clinical effect. In the cold-shower trial both groups cut depressive symptoms by 24%, so the cold package added nothing measurable over warm water and slow breathing.Source 1 · Randomized trialA randomized controlled clinical trial of a Wim Hof Method intervention in women with high depressive symptomsBlades R, Mendes WB, Don BP, et al.. DoseDoes the studied protocol match how people use it?
8/25The trials used daily cold showers for three weeks and an eight-session outdoor swimming course, and both sit close to what people do.Source 1 · Randomized trialA randomized controlled clinical trial of a Wim Hof Method intervention in women with high depressive symptomsBlades R, Mendes WB, Don BP, et al.. The claim drifts from the data here
Calling something a treatment means it has beaten a comparison in people with the condition. The one randomized test with an active comparison found that warm showers and slow breathing did as well as the cold package, and its participants were otherwise healthy women with high symptom scores.Source 1 · Randomized trialA randomized controlled clinical trial of a Wim Hof Method intervention in women with high depressive symptomsBlades R, Mendes WB, Don BP, et al.. Put it to use
If you have depression or anxiety and cold water helps you feel better, keep your existing treatment going alongside it. No trial supports swapping one for the other, and trials of cold water routinely exclude people with heart disease, so talk to your doctor first if that applies to you.Source 10 · Randomized trialThe Effect of Cold Showering on Health and Work: A Randomized Controlled TrialBuijze GA, Sierevelt IN, van der Heijden BCJM, et al.. “A cold plunge lifts your mood.”
People usually feel better right after a cold dip, and in the one randomized comparison of durations, five minutes lifted mood as much as twenty. None of the studies blinded people or used a placebo, so expectation can't be separated from the cold.

“Sauna treats depression.”
One session of medical whole-body heating eased depression more than a sham session in a trial of 29 people. When a pilot trial added infrared-dome heat to talk therapy, it did no better than sham heat.

“The Wim Hof Method lets you control your immune system.”
Men trained in the method had a smaller inflammatory response to an injected bacterial toxin, and the breathing did most of the work. Cold training alone left that response unchanged in a later trial, and no study has followed practitioners through real infections.
